After-Hours Questions
There is always an on-call provider available by phone. Please call your clinic number where an answering service representative will put you in contact with an on-call provider.

Important appointment considerations
New Patients: please arrive 30 minutes prior to your appointment.
Established Patients: please arrive 15 minutes prior to your appointment.
Patients who arrive late may be asked to reschedule.

Same Day Appointments
If you don’t feel well or have a more urgent need, we will make every effort to see you that day. Call any of our clinic locations to inquire about urgent provider availability. As a same-day appointment, please know that your wait may be longer and that your preferred primary care provider may not be available.

Payment for Services
CCHCI accepts ALL insurance plans. This includes all commercial plans, AHCCCS and Medicare. A Sliding Fee Discount Program is available for those who qualify. Self-pay incentives are also available. Co-payments and office charges are payable at time of service and can be taken care of with cash, personal check, debit or credit card at our clinic locations. For questions concerning your billing statements, please contact our Business Office.

Copayments are payable at the time of service.
